STATE FAIR Lady A #SMILEPOWER CONTEST RULES

The Delta Dental of Iowa State Fair Lady A #SmilePower Contest (the "Contest") is sponsored by Delta Dental of Iowa ("Delta Dental"). Contest entries will be accepted from August 20, 2016 at 8:00 am through August 20, 2016 at 3:45 pm, Central Time (the "Contest Period"). By submitting an entry, each entrant agrees to the terms included herein (the "Official Rules") and warrants that his or her entry complies with all requirements of the Official Rules.

WHO MAY ENTER

NO PURCHASE NECESSARY. A PURCHASE WILL NOT INCREASE THE CHANCES OF WINNING. The Contest is open only to individuals who are residents of Iowa and at least 18 years of age. Employees of Delta Dental and their respective corporate affiliates, successors and/or assigns, subsidiaries and agents, advertising and promotion agencies involved in any way with this promotion and members of their immediate family (spouse and parents, siblings and children of their spouse) and persons living in the same household of such individuals are not eligible to participate or win a prize. Limit one entry per person per day.

HOW TO ENTER

All entries must be submitted through this form and must include entrant’s name, mailing address, email address and mobile phone number (optional).

CONTEST PRIZES

The winner will be randomly selected on August 20, 2016 at 4:00 pm by a representative of Delta Dental. The Contest winner will receive two tickets to the Iowa State Fair's Lady Antebellum grandstand concert on August 21, 2016. The retail value for this prize is approximately $200. Delta Dental reserves the right to substitute the Prize in the event of unavailability. Delta Dental will notify the winner via email, text or phone (depending on the contact method chosen). The winner must claim the tickets within 30 minutes of being contacted. If a winner fails to claim the tickets within 30 minutes, the prize will be forfeited and may be awarded to an alternate winner.

SELECTION PROCESS

Using a random number generator, a winner will be selected from the contest entry sheet by a representative of Delta Dental of Iowa.

LIMITATION OF LIABILITY AND INDEMNIFICATION

By participating, each entrant agrees to release, discharge and hold harmless and indemnify Delta Dental and its affiliates, and their agents, employees, officers, directors, and representatives, from any claims, losses, damages, and expenses (including attorney fees) arising out of the entrant’s participation in this Contest or any Contest-related activities and the acceptance and use, misuse, or possession of any prize awarded hereunder. Delta Dental is not responsible for any technical problems with the Contest or technical malfunction of any computer systems or providers, software, the failure of any entry to be received on account of technical problems, human errors of any kind, or any combination thereof related to or resulting from participation in the Contest.

LICENSE

Each entrant grants Delta Dental an irrevocable, nonexclusive license to use all winning and non-winning entries (including name and likeness of entrant) for advertising, marketing, and promotional purposes, in any and all media now known or hereafter created, at Delta Dental’s discretion, without additional consideration, notification, permission to entrant or any third party.

CONDITIONS

THIS CONTEST IS VOID OUTSIDE IOWA AND WHERE PROHIBITED. Entrants agree that this Contest shall be subject to and governed by the laws of the State of Iowa and the forum for any dispute shall be in Des Moines, Iowa. Certain restrictions may apply. Entries are void if Delta Dental determines that an entry is illegible, incomplete, damaged, irregular, altered, counterfeit, produced in error, obtained through fraud or theft or does not otherwise meet the eligibility criteria outlined above. Entrants agree to be bound by these Official Rules and agree that the decisions of Delta Dental are final on all matters relating to the Contest. All federal, state and local taxes, fees and surcharges on prizes are the sole responsibility of the prize winners. Delta Dental reserves the right, at its sole discretion, to disqualify any entrants and/or to cancel, terminate, modify or suspend the Contest at any time.

DATA PRIVACY

Each entrant agrees that personal data, especially name and address, may be processed, disseminated or shared and otherwise used for the purposes and within the context of the Contest and any other purposes outlined in these Official Rules. The personal data also may be used by Delta Dental to verify the entrant’s identity and contact information or to otherwise verify the entrant’s eligibility to participate in the Contest. Entrants may also receive emails from Delta Dental about oral health and dental coverage. Entrants have the right to access, review, rectify or cancel any personal data collected in connection with this Contest by writing to Delta Dental at mhoneyman@deltadentalia.com. Any cancellation shall apply on a prospective basis starting on the date Delta Dental receives it.
